We are open for applications for individuals working in primary care, neighbourhood teams, and in voluntary, community and social enterprise (VCSE) organisations that are directly involved in the shift from hospital to community‑based care.
Individuals working in secondary care who believe they meet the eligibility criteria are also welcome to apply; however, priority will be given to applicants working in primary care.
This is a part‑funded programme. NHS England will contribute 50 per cent (£6,896) of the cost, with the remaining 50 per cent to be funded by the learner or their organisation.
